Filing Medical Insurance Claims |
Follow these 6 tips to help file a successful medical insurance claim:

Keep your current insurance ID card on-hand. |

|
Contact your insurance carrier and the medical provider in advance to confirm you will be covered. |
|
|
Confirm if your medical provider is in-network or out-of-network.
- In-network providers will typically file your claims for you.
- Out-of-network providers may file the claim as a courtesy, but are not required to do so.

|
|
Complete and return all necessary forms to your insurance carrier within the time permitted.
- Be sure to obtain the proper claim form from the carrier. You can do so by contacting ADPIA or visiting carrier website.
- Complete all fields of the form, as missing information can delay processing.
- Include the providers invoice for each service rendered including:
- Description of service
- Date of service
- Service code (cpt-4 code)
- Diagnosis (icd-9 code)
- Amount charged

When speaking with a medical provider’s office, or insurance carrier, take notes. |

If a claim is denied, file an appeal. |
