TIP OF THE MONTH: Filing Medical Insurance Claims

Follow these 6 tips to help file a successful medical insurance claim:

Keep your current insurance ID card on-hand.

Contact your insurance carrier and the medical provider in advance to confirm you will be covered.

Confirm if your medical provider is in-network or out-of-network.
  • In-network providers will typically file your claims for you.
  • Out-of-network providers may file the claim as a courtesy, but are not required to do so.

Complete and return all necessary forms to your insurance carrier within the time permitted.
  • Be sure to obtain the proper claim form from the carrier. You can do so by contacting ADPIA or visiting carrier website.
  • Complete all fields of the form, as missing information can delay processing.
  • Include the providers invoice for each service rendered including:
    • Description of service
    • Date of service
    • Service code (cpt-4 code)
    • Diagnosis (icd-9 code)
    • Amount charged

When speaking with a medical provider’s office, or insurance carrier, take notes.

If a claim is denied, file an appeal.
